Uses of Cisapride
Cisapride is prescribed to treat the symptoms and lesions caused by the return of gastric contents to the esophagus, to improve the side effects of gastric emptying due to diabetes, systemic sclerosis, autoimmune neuropathy, and to treat dyspepsia.
Mechanism of action of Cisapride
This drug releases acetylcholine from the cholinergic nerves in the gastrointestinal tract. Thus, this drug increases the activity and contraction of the lower esophageal sphincter and accelerates gastric emptying, which shortens the passage of intestinal contents.
Pharmacokinetics of Cisapride
Absorption of this drug is rapid and complete. The drug is metabolized by the liver and excreted by the kidneys and feces.
Contraindications to Cisapride
It should not be used in cases where gastrointestinal irritation is dangerous (such as bleeding, blockage or perforation of the gastrointestinal tract).
Side effects of Cisapride
Central nervous system: headache
Skin: Skin rash
Gastrointestinal: abdominal cramps, diarrhea, indigestion, bloating, nausea, dry mouth
Respiratory: rhinitis
Cardiovascular: Tachycardia
Central nervous system: anxiety, drowsiness, extrapyramidal reaction, fatigue, insomnia, seizures
Hematology and oncology: aplastic anemia, granulocytopenia, leukopenia, pancytopenia (reduction of all blood cells), thrombocytopenia
Liver: Increased liver enzymes
Infectious: Viral infection
Respiratory: cough, sinusitis, upper respiratory tract infection

Reducing the effects of drugs with cisapride:
Phosphomycin, Sincalide
Reducing the effects of cisapride by drugs:
Anticholinergic agents, opioid agonists
Food interference:
Concomitant use of the drug with grapefruit juice increases the bioavailability of the drug.
Solution: Minimize the simultaneous consumption of grapefruit or grapefruit juice with cisapride. The patient should be monitored for toxic effects of the drug, such as prolonged QT intervals and ventricular arrhythmias.
Precautions Cisapride
- The dose of this drug in patients with renal or hepatic impairment at the beginning of treatment should be half the usual amount.
- This drug should be used with caution in elderly patients or patients with heart disorders.
Cisapride drug recommendations
- In liver and kidney failure, the dose should be halved.
- It is recommended that this medicine be taken 15 minutes before a meal and at bedtime (in case of nocturnal symptoms) with a glass of water or other beverage.
